• Book your appointment and store important health information for note taking

• So we are able to get in contact with you in regards to appointments

Besides our staff, we share this information with:

• Cliniko and online notes system in order to keep notes online, private and password protected.

Providing some information is optional. If you choose not to enter health and contact details, we'll be unable to provide a health appointment.

We keep your information safe by storing it in password locked files and only your practitioner is able to access this information unless previously agreed by you to send this information in referral.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
